| 2. Grama Niladari shall send a report to the registrar of the area.. | |

| 4. Duly filled declaration of birth shall submit to the registrar of births and deaths of the area. | |
| 5. Declaration of birth can be obtained from the registrar. | |

| Registration of a Birth Occurred in a Private Hospital | 1. The Birth is registered by the Registrar of births and deaths of the area where birth occurred. |
| 2. If the birth has occurred at a private hospital, inform the Registrar of that area for registration of such birth. | |

Obtain a Certified Copy of the Certificate of Birth
Required Forms- Application
| Obtain a Certified Copy of the Certificate of Birth | 1. Refer the application to the Divisional Secretariat of the area in which the place of birth is occurred. |
| 2. Application can be obtained from any Divisional Secretariat. | |
| 3. Fees chargeable | |
| I. If the number of the certificate of birth & the registered date is known Rs.100.00 | |
| II. If don't know the number of the certificate of birth or the registered date, | |
| III. වfor search of birth registers for period of 02 years Rs.200.00 | |
| 3. Required fee should be paid by currency. | |
| 4. Duly filled application should be submit to the Divisional Secretariat. | |
| 5. If you like to obtain the birth certificate by post, submit a stamped envelop with the application. | |
| 6. If the birth is registered a birth certificate will be send and if the birth is not registered a letter to the effect will be send. | |
| 7. Any Number of copies can be obtain by one Application(Circular-02/2012) |
Alteration of Information in a Birth Register
| Alteration of Information in a Birth Register | 01.In a Birth certificate, |
| I. Information in cage 01 (Date & place of birth) | |
| II. Information in cage 03 (Sex) | |
| III. Information in cage 05 (Information of mother - if mother's name has been changed after the registration of the birth, it is not possible under this provision) | |
| IV. Information in cage 09 (Informant's information) may be altered | |
| 02. Produce the declaration to the Additional District Registrar of the Divisional Secretariat of the area where birth occurred. | |
| 03. The declaration should be produced by, | |
| I. Owner of the birth certificate | |
| II. Father or Mother | |
| III. Legal guardian | |
| IV. A person who is dissatisfied with any of the information appearing in the birth register | |
| 04. Rs.50.00 should be paid by currency for the declaration | |
| 05. A certified copy of the birth register to be amended should be essentially attached to the declaration. | |
| 06. Following are some written evidences which may be produced to substantiate the request made in the declaration, | |
| I. Parents' marriage certificate | |
| II. List of children/ brothers and sisters | |
| III. Birth certificate of father/ mother | |
| IV. Birth certificate of a child born after the registration of parents' marriage | |
| V. In case there is no child born after the registration of the marriage of parents, birth certificate of a elder or younger child of the subject. | |
| VI. Certified copies of records available in hospitals, maternity homes or midwife's records | |
| VII. Student's record sheet, extract of the school admission register | |
| VIII. Copy of the electoral register where names of the subject and parents are included | |
| IX. If father is deceased, his death certificate |
Registration of Past Births
| Registration of Past Births | 01. If a birth has not been registered with 03 months since its occurrence it may be registered.. |
| 02. Declaration should be produced to the Additional District Registrar of the Divisional Secretariat where birth occurred. | |

| 1. Marriage certificate of parents if they are married | |
| 2. One of following documents in proof of the date and place of birth of the subject. | |
| I. Hospital birth report or certified copy | |
| II. Grama Niladhari birth report or certified copy | |
| III. Estate birth report or certified copy | |
| IV. Public Health Officer's certificate | |
| 06. Following documents should be produced with the declaration produced after one year since occurrence of the birth | |
| I. Marriage certificate of parents if they are married | |
| II. Nil result sheet | |
| III. One of the following documents indicating date and place of birth of the person | |
| i. Hospital report of certified copy | |
| ii. Grama Niladhari report or certified copy | |
| iii. Estate birth report or certified copy | |
| iv. Copy of the Midwife's birth register | |
| IV. In case of non-possession of any of the above documents, | |
| a. Health development record | |
| b. Certificate of baptism | |
| c. Certified copy of the school admission register / pupil's record sheet. | |
| d. Estate welfare officer's report | |
| e. Horoscope of the record of time prepared on a proximate date to the day on which the birth occurred | |
| V. If any of these documents are not available, declaration including details having knowledge of and an affidavit. |
